Clinical Care Support Specialist (LPN)

In this role, you’ll provide hands-on clinical support, health advocacy, and compassionate care for sisters living in Providence Hall and nearby residences. You’ll work closely with the Clinical Care Director, assist with assessments, medications, care coordination, and occasionally accompany sisters to appointments — serving as a trusted advocate every step of the way.

What You’ll Do

Working closely with the Clinical Care Director, you will support the physical, emotional, and overall well-being of sisters by:

  • Providing basic clinical care such as taking vital signs, conducting basic health assessments, occasional medication administration, filling pill planners, facilitating medication refills, and simple wound or personal care,
  • Serving as a health advocate by assisting with communication between sisters and healthcare providers, discharge planners, home health agencies, and community-based services,
  • Occasionally transporting and accompanying sisters to medical appointments when advocacy is needed,
  • Assisting with coordination of intermittent ancillary services (e.g., podiatry),
  • Completing occasional data entry and related paperwork,
  • Helping maintain, restock, and organize medical supplies and equipment, and
  • Performing other clinical and clerical tasks as needed.

What We’re Looking For

Qualifications and Skills:

  • Strong understanding of aging and the holistic needs of older adults,
  • Comfort and experience working in a clinical care setting,
  • Professional, empathetic, and compassionate communication skills,
  • Patience and sensitivity when working with individuals who may be ill, disabled, or experiencing loss,
  • Ability to remain calm and adaptable in urgent or changing situations,
  • Strong organizational and time-management skills,
  • Proficiency with Microsoft Word, Excel, and Google applications,
  • Commitment to confidentiality and HIPAA compliance,
  • Supportive of the mission and values of the Sisters of Providence, and
  • Valid driver’s license with a good driving record; ability to travel locally in Terre Haute as needed.

Education and Experience

  • Graduate of an accredited School of Nursing,
  • Active LPN license in good standing in the State of Indiana,
  • Minimum of 3 years of experience in geriatric care, home health, or long-term care, and
  • Experience supporting the physical, mental, and spiritual well-being of older adults.

Physical Requirements and Work Environment

  • Combination of office work and walking throughout Providence Hall and nearby buildings,
  • Occasional travel with sisters to and from medical appointments,
  • Ability to assist with entering and exiting vehicles, and
  • Ability to lift and maneuver wheelchairs, walkers, and medical equipment.
